We are required by law to:

  • Maintain the privacy of your child's protected health information
  • Provide you with this Notice of our legal duties and privacy practices
  • Notify you if we are unable to agree to a requested restriction
  • Follow the terms of this Notice currently in effect
  • Notify you if a breach occurs that may have compromised your child's health information

Changes to This Notice

We reserve the right to change this Notice and make the revised Notice effective for health information we already have as well as any information we receive in the future. We will post the current Notice in our office and on our website.

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
