Call (480) 892-3880 to schedule your child's first appointment. Our staff will help you choose the best time and provider.

Complete Paperwork

Fill out the new patient forms before your visit or arrive 15 minutes early to complete them at our office.

Transfer Records

Request medical records and immunization history from your previous provider to be sent to our office.

Arrive for Your Visit

Bring all required documents and arrive on time. Check in at the front desk and we'll take care of the rest!

What to Bring

To ensure a smooth first visit, please bring the following items to your appointment:

  • Insurance card (front and back)
  • Photo ID of parent/guardian
  • Immunization records from previous provider
  • Medical records (if available)
  • List of current medications
  • Completed new patient forms

What to Expect at Your First Visit

Check-In

Arrive a few minutes early to complete any remaining paperwork. Our friendly front desk staff will verify your information and insurance.

Comprehensive Exam

Your provider will perform a complete physical examination, review your child's medical history, and address any health concerns you may have.

Discussion & Questions

We encourage questions! Your provider will discuss your child's health, answer your questions, and develop a care plan together.

Prenatal Consultations

Expecting your first child? We offer prenatal consultations for expecting parents who want to meet our team and learn about our practice before their baby arrives.

During a prenatal visit, you can tour our office, ask questions about newborn care, and get to know the providers who may care for your baby.
